VETERINARY MEDICINE AND ALLIED PRODUCTS DIRECTORATE (VMAP)
GUIDELINES FOR WAREHO– USE INSPECTION.
- These Guidelines are for the interest of the General public and companies engaged in importation of animal feeds, feed additives, feed concentrates, feed supplements, premixes and fish meal.
- It is necessary to emphasize that no Animal feeds, Feed additives, Feed concentrates, Premixes and fish meal shall be imported, distributed, sold or advertised, in Nigeria except in accordance with the provisions of NAFDAC ACT CAP N1 LFN 2004.
B. APPLICATION FOR WAREHO– USE INSPECTION
An application for warehouse inspection shall be made by the importer on company’s letter head and addressed to:
The Director General
National Agency for Food and Drug Administration and Control (NAFDAC)
ATTN: The Director, Veterinary Medicines and Allied Products Directorate
No 445, Herbert Macaulay way, Yaba, Lagos.
C. DOCUMENTS REQUIRED FOR BOOKING WAREHO– USE INSPECTION
The following documents are to be submitted:
i. Current medical certificate of fitness of Technical officer and other staff in the warehouse.(not more than six month)
ii. Copy of the previous year’s import permit (if applicable)
iii. Receipt/ stock records (BIN cards) for previous year’s importation, local purchase order and distribution(if applicable)
iv. Invoice/waybills/delivery notes of materials sold or purchased locally.
v. Photocopy of payment receipt of rent or proof of ownership of warehouse.
vi. Record of waste disposal in a timely and sanitary manner.vii. Evidence of fumigation / Pest control
viii. SOP for cleaning and maintenance of warehouse in a sanitary manner.
ix. Copy of registration certificate of regulated products (if any)
x. Names and addresses of distributors (Toll millers and distributors)
- Please note that company’s Technical officer must be present for the inspection.
- All documents must be submitted in duplicates before inspection.
All payments to the Agency should be in Bank Draft /cash, payable to National Agency for Food and Drug Administration and Control (NAFDAC) account at designated bank(s).
FEE CHARGED FOR INSPECTION:
a. Inspection- N10,000
b. VAT (5%)- N 500
Grand Total- N10, 500
NOTE: FAILURE TO RESPOND TO CONCERNS RAISED BY NAFDAC ON THE APPLICATION, WILL AUTOMATICALLY LEAD TO DELAY IN FURTHER PROCESSING OF THE APPLICATION.
